چکیده

Purpose: To report a case of double descemet membrane forming a double anterior chamber following ‘Big bubble’ DALK converted to PKP. Results: A 52-year-old man with keratoconus underwent DALK operation converted to PKP due to macro perforation of the descemet membrane. On the postoperative evaluation a remnant of the host’s descemet membrane formed a double anterior chamber. After a few months with no intervention, an attempt to deflect a thickened descemet membrane had failed, making entrance into the anterior chamber in order to excise the membrane inevitable. Two years following the second surgery a graft endothelial failure and cataract formation was documented, leading to a second corneal transplantation and lens implantation. Conclusion: Double DM is an unusual condition following a DALK procedure which was converted into PKP. In the above case this complication resulted in multiple procedures and graft failure. We believe that early interventions such as gas injection or YAG laser could have resulted in a more favorable outcome.

Spontaneous resolution of Descemet membrane detachment following big-bubble deep anterior lamellar keratoplasty.

PURPOSE To report 2 cases of delayed spontaneous Descemet membrane (DM) reattachment following big-bubble deep anterior lamellar keratoplasty (DALK). METHODS A 21-year-old man with keratoconus and a 36-year-old woman with corneal macular dystrophy underwent big-bubble DALK. No injury to DM was noted intraoperatively or postoperatively. Postoperatively, the patients developed central DM detach...

Double-bubble technique to facilitate Descemet membrane exposure in deep anterior lamellar keratoplasty.

Safe and efficient exposure of Descemet membrane is the key to success in deep anterior lamellar keratoplasty. Although widely used, the big-bubble technique has the drawback of difficulty in maintaining appropriate needle insertion depth in the corneal stroma, resulting in injected air sometimes escaping to the peripheral cornea without separation of Descemet membrane. We describe a variation ...

An unusual case of acute angle-closure glaucoma following deep anterior lamellar keratoplasty using the “big bubble” technique

Purpose To report the first case of acute angle closure due to a high-pressure Descemet membrane detachment following deep anterior lamellar keratoplasty (DALK) using the "big bubble" technique. Observations A 25-year-old man underwent DALK surgery for keratoconus using the "big bubble" technique in which an air bubble is injected in deep stroma to promote dissection of underlying Descemet me...

Spontaneous reattachment of Descemet membrane detachment after deep anterior lamellar keratoplasty

RATIONALE To report a case of late spontaneous reattachment of Descemet membrane (DM) detachment after deep anterior lamellar keratoplasty (DALK). PATIENT CONCERNS A 50-year-old woman with stromal corneal scarring underwent a DALK with the big bubble technique. The DM detachment and double anterior chamber were observed on the first day after operation. DIAGNOSES Descemet membrane detachmen...

Stripping of Descemet's membrane while refilling the anterior chamber.

A 67-year-old man underwent a trabeculectomy for pseudo-exfoliative glaucoma. The postoperative course was complicated by overfiltration with hypotony and a flat anterior chamber (AC) with lens touch. While the AC chamber was refilled at the slitlamp with a viscoelastic (Healon), stripping of Descemet’s membrane was observed. The patient was referred to our department. Slitlamp examination show...
